21st Japan Flute Convention in KAWASAKI 2023 Application Form

This is the application form for members of the JFA or General Attendee. If you wish to join and participate simultaneously, please apply using this application form. Flow of Convention Registration:

  • Please fill out this form and click the “Submit” button.
  • If your email address is correct, you will receive an email with the subject line “<21st Japan Flute Convention in KAWASAKI 2023> Application received” . If you do not receive this email, there is a possibility that your email address was not entered correctly, or emails from Japan may be blocked. If you are unable to proceed successfully, please contact us directly via email.
  • After receiving your submission in the system, we will send you an email titled “Notification of Convention Registration Receipt and Request for Payment of Participation Fee” with information on the participation fee and payment instructions. For association members, we will check your membership status based on your membership number, so please be sure to enter your membership number. If you wish to join as a member simultaneously, please apply using this application form.
  • Once your payment is confirmed, we will send you an email with the subject line “Notice of Receipt of Conventions Participation Fee” containing a “QR code” to exchange for the “Participation Pass” on the day of the convention. This QR code will be scanned at the reception desk on the day of the event. If you have a smartphone, you do not need to print it out as long as it can be displayed on your screen. If you do not have a smartphone, please print out the QR code at home and present it at the reception desk on the day of the event.
  • If you are a guest performer at the conventions, please indicate the name of the performing organization in the comments section.

    Venue: Hotel Morino Shinyurigaoka 7F "Hinoki no Ma" (1 minute walk from South Exit of Shinyurigaoka Station on Odakyu Line)
    Participation Fee: 8,000 yen (9,000 yen on the day)
